Our Hospital to Home program begins before discharge. We’ll meet with you and your care team to understand clinical needs, preferences and goals. A personalised care plan is then developed, covering both immediate and ongoing support needs.
We assist with transport home and ensure the essentials are in place for a safe arrival — such as food, medication pick-up, and a clean, accessible home environment.
Once home, we provide care that adjusts as recovery progresses. From personal care and household assistance to rehabilitation support and emotional encouragement, our team is by your side through it all.
Consistent Support, Every Step
Clients recovering at home often need reminders for rehabilitation exercises or support getting to medical appointments. Our carers offer gentle, practical assistance with walking, transferring, and safe movement around the home. We also accompany clients to physiotherapy, GP reviews, and community or family events when desired.
Personal care needs are managed with dignity — including bathing, dressing, continence support and grooming. Medication reminders and wellness monitoring are part of every care shift, ensuring nothing is missed and any changes are promptly addressed.
